Job summary

Paycom - ATS is seeking an ALF Case Manager to coordinate care for ALWP clients under the Care Coordination Agency in California. You will assess eligibility, schedule monthly visits, verify Medi-Cal and coordinate DME, appointments, and medication adherence while maintaining accurate Devero records.

Responsibilities include communicating with facility staff, tracking SIRs, and ensuring ISP sign-off and documentation within seven business days, with driving across LA and surrounding counties as

Qualifications

• Bachelor’s Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology; plus one year of related work experience.
• Current CPR certification.
• Knowledge of California Assisted Living Waiver Care Coordination Agency preferred.
• Experience in education or training, quality assurance and management preferred.
• Excellent judgment and knowledge of current clinical practices, with strong documentation skills.
• Good organizational skills and ability to work with little supervision.
• Proficient in computers and electronic information systems.
• Ability to establish and maintain good communication with office, field, and administrative personnel.
• Familiarity with Title 22 preferred.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Check Medi‑Cal eligibility for each client monthly.
• Schedule monthly visits with clients.
• Assess medication adherence and needs such as DME and appointments.
• Coordinate with staff and track SIRs; ensure SIR copies are obtained.
• Update client information in Devero and upload notes by the next business day.
• Notify the ALF Coordinator when a client leaves and returns to the facility.
• During re-assessments, copy ISP, have client sign, and upload to Devero within 7 business days.
• Maintain confidentiality and follow HIPAA and healthcare regulations.
