Responsibilities

• Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs; reviews patient history and relevant data; communicates findings.
• Formulates and initiates a plan of care including discharge planning; collaborates with patient/family and health team.
• Implements the plan of care through direct patient care, coordination, delegation, and supervision of the health care team.
• Evaluates patient and family response to care and teaching; revises plan of care as needed.
• Documents assessment, planning, implementation, and evaluation in patient records with clear communication.
• Promotes evidence-based practice and participates in education and leadership within the unit.
• Contributes to improving patient outcomes and supports continuous quality initiatives.
