Horizon House, Inc. is seeking a Part-Time Aide to support individuals in a residential setting who are experiencing mental health crises but do not pose immediate risk.
You will assist clients to live and function independently while participating in developing and implementing their individual program plans, under supervision. Responsibilities include admission documentation, orienting participants, monitoring health changes, and supporting treatment goals.
We are currently looking for a Part-Time Aide to provide support in a residential setting to individuals experiencing mental health crises who do not pose an immediate risk to their own or the safety of others, and assist individuals with learning to live and function as independently as possible in the community. facilitate participation in the development and implementation of individual program plans, and in the coordination and arrangement of specialized service provision. Receives general and regular supervision from the Site Manager, and may receive additional support and guidance from Mental Health Professionals.
