Service Area: MetroWest, Worcester County, and parts of Blackstone Valley Location: Remote with travel to client locations
Flexible Hours: 15-25 hours/week is the final goal. Start low (5+ hours/week) and grow with us!
Compensation: Hourly for billable time and select non-billable activities such as training, educational events, team meetings.

The Role:
As an Aging Life Care Professional at The Aging Space, you will be a steady, trusted guide for clients and their support system — navigating care options, building thoughtful plans of care, and offering support and education that is both personal and professional.
You will:
Build real relationships with clients and their support system
Conduct comprehensive, holistic assessments
Develop and manage personalized plans of care
Coordinate care across medical, home, and community settings
Advocate for your clients’ wishes as well as their dignity, safety, and quality of life
Communicate clearly and compassionately — in writing and in person
Use care management software and shared digital tools
Collaborate with our team while working independently in the field
What You Bring:
Bachelor’s degree in healthcare or a related field (Master’s preferred)
Active licensure (RN, LCSW, OT, PT, SLP, or similar) and personal professional liability insurance
2–4 years’ experience working with the aging population
Background in adult day health, assisted living, home care or skilled nursing
Exceptional professional written and verbal communication
Experience completing holistic assessments and creating care plans
Strong documentation, planning, and time management skills
Comfort and experience using various technologies including smart phones, laptops/computers and tablets.
Proficiency in Outlook (email and calendar), Word, Excel, shared folders, and care management software
A valid driver’s license, a work use clause in your auto insurance and a willingness to drive 45+ minutes as needed
A warm, flexible, “can-do” mindset — ready to say “yes” and figure things out
Curiosity, initiative, and a genuine desire to grow both personally and professionally
A true team player who values collaboration and believes in lifting others up
