At The Aging Space, we believe aging and/or illness is different for each individual. We walk alongside our clients and their support system during some of life’s most difficult and confusing times. We offer high-touch, compassionate care management with the goal of bringing peace of mind, ensuring dignity, and creating joy for all involved.

We truly listen to our clients’ wishes and create personalized plans that help them live their best life even if they are living with cognitive and/or physical limitations.

We strive to be known throughout the community for our warmth, honesty, expertise, flexibility, and can-do attitude — not just by our clients, but by their support system, our care partners, and each other. We aim to be a company that people love working with and at.

Above all else, we make sure no one walks their path alone.

Our Culture:

We are warm, compassionate professionals with deep experience and education in aging and chronic illness care.

We:

Prioritize being available to clients during work hours

Think creatively to solve problems with empathy and skill

Work both independently and collaboratively to meet client needs

Respect the individuality of our clients and each other

Find joy and purpose in making life better for others

Value each team member’s unique voice and expertise

Know that supporting our clients — and one another — is what drives real success

The Role:

As an Aging Life Care Professional at The Aging Space, you will be a steady, trusted guide for clients and their support system — navigating care options, building thoughtful plans of care, and offering support and education that is both personal and professional.

You will:

Build real relationships with clients and their support system

Conduct comprehensive, holistic assessments

Develop and manage personalized plans of care

Coordinate care across medical, home, and community settings

Advocate for your clients’ wishes as well as their dignity, safety, and quality of life

Communicate clearly and compassionately — in writing and in person

Use care management software and shared digital tools

Collaborate with our team while working independently in the field

What You Bring:

Bachelor’s degree in healthcare or a related field (Master’s preferred)

Active licensure (RN, LCSW, OT, PT, SLP, or similar) and personal professional liability insurance

2–4 years’ experience working with the aging population

Background in adult day health, assisted living, home care or skilled nursing

Exceptional professional written and verbal communication

Experience completing holistic assessments and creating care plans

Strong documentation, planning, and time management skills

Comfort and experience using various technologies including smart phones, laptops/computers and tablets.

Proficiency in Outlook (email and calendar), Word, Excel, shared folders, and care management software

A valid driver’s license, a work use clause in your auto insurance and a willingness to drive 45+ minutes as needed

A warm, flexible, “can-do” mindset — ready to say “yes” and figure things out

Curiosity, initiative, and a genuine desire to grow both personally and professionally

A true team player who values collaboration and believes in lifting others up

You’ll Love Working with Us If You:

Love working with older people

Believe aging should be full of dignity, support, purpose, meaning and joy

Enjoy developing deep family like relationships with your clients

Are organized, independent and love being part of a thoughtful team

Want to grow in a supportive, mission-driven environment

Understand that team success fuels individual growth and success — not the other way around

Thrive when you’re helping others thrive

What We Offer

Flexible part-time hours, up to 25/week that expand as our client base grows

Hourly compensation for billable time and select non-billable time for training, meetings, educational events and networking

Membership in ALCA, our national association, and support toward earning your care management certification

A deeply supportive team that values your ideas and input

A workplace built on trust, warmth, collaboration, and integrity

We lead with heart. We act with integrity. And we’re always in it for the right reasons — our clients, our community, and our team.

If you’re the kind of person who brings a “Yes, I can!” attitude, thrives on connection, and finds meaning in making life better for others, you might be exactly who we’re looking for.
