Job summary

Symphony Care Management, LLC is seeking an Aging Life Care Manager in Greater Boston. The role focuses on clinical eldercare, coordinating care across homes, facilities, and hospitals to support older adults and families through transitions.

You will assess clients, plan care, coordinate services, advocate, and coach caregivers, traveling to homes and facilities as needed, with a flexible schedule and strong reimbursement in a busy community setting.

Qualifications

  • College degree in social work, nursing, or allied disciplines; Master’s degree preferred.
  • Minimum five years’ experience with an older adult population.
  • Valid driver’s license, auto insurance, clean driving record, reliable private vehicle.

  • Perform client assessments
  • Interact with clients/families with non-judgmental compassion and competence
  • Create and assist in coordination and implementation of the care plan
  • Oversee and anticipate ongoing client needs
  • Maintain ongoing communication and collaboration with health care providers
  • Continually evaluate client goals and steps to reach them
  • Complete accurate, concise, timely documentation within 48 hours
  • Actively participate in weekly supervision and team meetings
  • Be willing to drive up to 40 minutes to a client’s home or appointment

Position Title: Aging Life Care Manager

Job Description: We are seeking a professional with a wide range of clinical experience in elder care in environments such as a client’s home, assisted living, retirement communities, hospitals, hospice care and skilled nursing facilities as well as knowledge and experience working with individuals with Alzheimer’s Disease and related disorders and mental health challenges. You will be responsible for client(s) assessment, creative client centered care planning, education, medical and service care coordination, advocacy, and caregiver coaching.

This position requires you to travel to our client’s homes, retirement communities, assisted living or nursing homes, and see clients in the hospital when needed. Our client population is predominately older, but not exclusively. Inherent in the position is working collaboratively with clients and/or families, physicians, and ancillary health providers with the goal of maintaining or promoting independence with appropriate high quality and cost-effective services.

Duties & Responsibilities:

  • College Degree in Social Work, Nursing or allied professional disciplines. Master’s degree preferred.
  • Minimum five (5) years’ experience with an older adult population
  • Valid Driver’s license, current auto insurance, clean driving record, and use of a reliable private vehicle
Requirements:
  • Excellent organization and time management skills.
  • Ability to use technology including Word Processing, Zoom, and Google Suite.
  • Three references from employers, including one from a direct supervisor.
  • Must be able to work within the construct of the client centered culture and core values.
  • Must be able to workcollaboratively with team members.
