Aging in Place Coordinator

Summary
Under supervision of the Assistant Director of the Senior Services program, coordinate all In-Community services of the older adult center network. Oversight of all funded and non- funded services provided to older adult participants.

Schedule
Monday - Friday 9AM - 5PM
Location
This position will be based out of Golden Phoenix Adult Center location 1 but may have to transport to other sites
Position Type
Core duties and responsibilities include the following. Other duties may be assigned as needed
- Maintains proper oversight of the stocking and re-stocking of supplies for the program.
- Coordinates the planning of in-community services such as:
- Benefits & entitlements
- Case Assistants
- Shopping Assistance
- Chore Services
- Friendly Visiting
- Telephone Reassurance.
- Manages a robust Evidence -Based program for Caregiver and Behavioral Health supports for families at home.
- Maintains and complies with the standards set by Department for the Aging (DFTA) and Department of Health & Mental Hygiene (DOHMH) including, city, state, federal governing guidelines.
- Ensures in-home, older adult safety as required by the Fire Department of New York (FDNY)
- Supervising and training of volunteers and staff of In-Home services
- Supervise In-Home Program Specialist(s) with their dutie
- Reports potential unsafe conditions to the Assistant Director in a timely manner
- Assures the accurate and up-to-date required documentation of records are kept in a neat and organized manner.
- Weekly observance of STARS data for accuracy.
- Supervises the daily input of case notes; and updating of senior information in the STARS system.
- Meets all deadlines of the assigned duties
- Activity Calendar for Homebound participants
- Monthly Units of Service data report
- Monthly In- Community program report
- Maintains and ensures that all unit targets and goals are met by staff.
- Submits all unit sheets documentation to a Data Specialist daily.
- Conducts weekly case meeting to properly follow-up items for seniors to efficiently manage in their homes.
- Create and maintain an accurate list of support linkage groups and collaborative contacts.
- Oversees that older adults are referred to virtual activities and other resources for social engagement.
- Manages all In-Community staff and programming across the Elmcor Network.
- Identifies and resolves problems in a timely manner for their department.
- Develops alternative solutions with the Assistant Director and/or the Director.
Qualifications

- Must receive/obtain a Food Protection License from the Department of Health & Mental Hygiene, and receive a copy of their State guidelines. These guidelines must be followed at all times.
- Must receive/obtain First Aid, CPR, AED training.
- Must receive/obtain Certificate of Gerontology
- Must have valid Driver’s License
- Must have a High School Diploma with 5+ years’ experience in senior services or related field in a managerial capacity
Supervisory Responsibilities
Directly supervise all volunteers and staff in the In-Community department. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws
