Job summary

Qualifications

Aging Case Manager

Aging & Social Services Department

Making a Direct Impact in the Lives of Older Adults

Are you passionate about supporting older adults and helping them maintain independence, dignity, and quality of life? Do you thrive in a role where compassion, advocacy, problem-solving, and real-world service coordination come together every day? As an Aging Case Manager at GRADD, you play a vital role in helping older adults receive appropriate, timely services that support their independence and well-being. Through in-home assessments, individualized care planning, service coordination, and advocacy, you help connect participants with the resources they need to live safely and with dignity in their communities. This is a meaningful, people-centered role where your work directly improves lives across the region.

Key Responsibilities Include
Assessment & Care Planning
  • Conduct in-home eligibility assessments and comprehensive evaluations of client needs.
  • Develop and implement individualized plans of care based on assessment findings.
  • Respond promptly to client referrals and inquiries.
Advocacy & Service Coordination
  • Advocate on behalf of participants with appropriate community resources.
  • Coordinate services with formal and informal providers to ensure continuity of care.
  • Make timely referrals to community agencies according to participant needs.
  • Maintain regular communication with participants, families, and service providers.
Collaboration & Communication
  • Work closely with the Aging Case Management Supervisor and other case managers to provide services.
  • Attend meetings and contact individuals involved in participants’ welfare.
  • Participate in staff development, job trainings, and meetings.
Documentation & Case Management
  • Maintain accurate, timely documentation of assessments, care plans, and case activity.
  • Provide ongoing case management support as assigned by the Aging Case Management Supervisor.
Other Responsibilities
  • Assist with special projects as assigned.
  • Support additional programs as time permits, which may include staffing committees or councils, providing community education, or utilizing professional credentials for chart reviews or other program support tasks.
Education & Experience
  • Bachelor’s or Master’s degree in Social Work, Gerontology, Psychology, Sociology, or a related field.
  • Bachelor’s or Master’s degree in Nursing with current Kentucky nursing license also accepted.
  • Reliable transportation required.
  • Broad knowledge of the needs of older adults and other client groups, as well as familiarity with community resources and service organizations.
Skills & Competencies
  • Strong interpersonal and communication skills, including with participants who may have physical or cognitive limitations.
  • Excellent written communication and computer skills.
  • Ability to coordinate multiple services and work independently in the field.
  • Compassionate, client-centered approach with attention to confidentiality and ethical standards.
