Job summary

Intent Clinical is seeking an Aging Care Manager in Naples, FL for a part-time, per circumstance role. You will assess and plan individualized care for aging clients, coordinating with families and healthcare providers to ensure high-quality support.

Requirements include a Master’s in a related field and active licensure, with at least five years’ direct care experience in care management or related settings. Strong communication and independence are essential.

Qualifications

• Master's degree in Social Work, Nursing, Gerontology, Occupational Therapy or related field.
• Professional Licensure in good standing (RN, NP, LMSW or other nursing/social worker license).
• Minimum of 5 years of direct care experience in care management, home care, acute care, or rehabilitation settings.
• Strong knowledge of aging issues, healthcare systems, and community resources.
• Experience working closely with families and professional providers.
• Ability to work independently and within a multidisciplinary team.
• Excellent written and verbal communication, interpersonal, and organizational skills.
• Comfort with technology (Microsoft Office, Salesforce) and valid driver's license.

Responsibilities

• Conduct client assessments and develop individualized care plans to meet goals.
• Coordinate communication between clients, families, providers, and services.
• Arrange medical, social, and personal care services, including home health and transportation.
• Advocate for clients' needs and preferences in all aspects of care.
• Identify community resources and assist with navigating benefits and entitlements.
• Provide education and support on aging-related issues and resources.
• Perform regular follow-ups, adjust plans as needed, and document interactions.
