Job description

This is a Care Management Supervisor I position within the Care Management Unit of the Area Agency on Aging for
Luzerne/Wyoming Counties. Employee in this position supervises a unit of Care Managers, and one Case Aide.
Employee is responsible for the Personal Emergency Response and Medical Supplies Programs.

  • Planning, organizing, and assigning all work to staff to ensure timely completion.

  • Providing training and guidance on policies and procedures in a prompt and knowledgeable fashion, and providing information to staff on appropriate resources available to the elderly.

  • Implementing procedures to conform with PDA program policies, including, but not limited to OPTIONS, Cost Sharing, SAMS/OMNIA, Family Caregiver Support, Domiciliary Care, and Care Management programs.

  • Monitoring, reviewing, and approving all paperwork and data entered into the agency database by care managers and analyzing for completeness and adherence to established standards.

  • Conducting regular case conferences with each care manager.

  • Conducting supervisory field visits.

  • Representing staff needs and concerns to administrative staff to ensure productive working relationships within the agency.

  • Specifying training needs of staff within the unit and arranging for appropriate training programs.

  • Making presentations to community groups regarding aging services.

  • This position is responsible for the Personal Emergency Response and Medical Supplies Programs.

Administrative Responsibilities:
Care Management:
• Makes presentations to community groups regarding aging services, as directed
• Serves on committees as directed
• Attends trainings and conferences, as directed; participates in Aging network activities
• Other duties as assigned

Personal Emergency Response Program Responsibilities:
• Serves as a liaison with providers of Emergency Response Services
• Arranges for installation/removal of emergency response machines
• Monitors usage and handles consumer/program problems
• Arranges for repairs of equipment
• Reviews monthly invoices for accuracy
• Monitors monthly program spending
• Manages waiting list
Medical Supplies:
• Serves as a liaison with providers of Medical Supplies
• Monitors usage and handles consumer/program problems
• Reviews monthly invoices for accuracy

  • Knowledge of social casework principles and methods
  • Knowledge of current social, economic, and health problems and community resources as related to older adults
  • Knowledge of individual and group behavior and ways of working effectively with older adults who have social, economic, emotional, and/or health problems
  • Knowledge of the basic principles and methods of program interpretation and community organization
  • Skill in the development of plans to address the needs of older adults
  • Ability to work effectively with older adults and aid them to grow in the constructive use of their potential in adjusting to their specific problems
  • Ability to evaluate work of subordinates and to provide technical assistance and guidance
  • Ability to orient and instruct subordinates on procedures, techniques, laws and regulations
  • Ability to establish and maintain effective working relationships with clients, other members of the staff, outside agencies and institutions, and the general public
  • Ability to clearly express ideas orally and in writing

ELIGIBILITY -ALL CANDIDATE{S):
1.) Meet the minimum experience and training (MET'S) required for this position
Three years of experience in public or private social work and a Bachelor's Degree; or any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.2.) Be a resident of Pennsylvania.
3.) Hiring preference for this vacancy may be given to candidates who live within Carbon, Columbia, Lackawanna, Luzerne, Monroe, Schuylkill, Sullivan, or Wyoming counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered. 4.) You must be able to perform essential job functions.

ELIGIBILITY - COMPETITIVE PROMOTION WITHOUT EXAMINATION ONLY: CLASS RESTRICTIONS:
1.) Have held regular civil service status in the following classifications:
  • Aging Care Manager 3
  • Aging Care Manager 2
SELECTION CRITERIA:
1.) Meet the minimum experience and training required for the job.
2.) Meritorious Service , defined as:
  • The absence of any discipline above the level of written reprimand during the 12 months preceding the closing date on the posting.
  • The last due overall regular or probationary performance evaluation was higher than unsatisfactory.
3.) Seniority , Defines: A minimum of twelve months in the next lower class(es) by the closing date of this posting MM/DD/YYYY.
