Job summary

Optum is seeking an Afterhours Registered Nurse to provide comprehensive hospice care after regular business hours. Responsibilities include triaging patient needs, responding to crisis calls, and conducting nursing evaluations. The ideal candidate will hold a current RN license in Louisiana and have at least two years of RN experience.

Qualifications

• Current driver's license and valid vehicle insurance.
• Minimum of two years full-time experience as an RN or two years as an LPN in hospice care.

Responsibilities

• Triage patient needs over the phone until a nurse can visit.
• Respond to crisis calls from patients and families after hours.
• Conduct initial nursing evaluations within 48 hours of referral.
• Perform afterhours visits as needed.
• Identify and reassess patient/family needs every 15 days.
• Document patient problems, goals, and responses.

As the Afterhours Registered Nurse your primary focus is to conduct comprehensive and timely delivery of hospice care for patients and their families after regular business hours, including at night and on the weekends.

Primary Responsibilities

• Able to triage patient needs over the phone until a nurse can provide an in-person visit
• Responds appropriately and with urgency to crisis calls from patients and families after hours
• Makes the initial nursing evaluation in determining eligibility for hospice services, as part of a patient visit, within forty-eight (48) hours of referral if assigned
• Competent to perform afterhours visits including admission visits, death visits, recertification visits, and routine visits
• Identifies the patient/family's physical, psychosocial, emotional, and environmental needs and reassesses them as needed, no less than every fifteen (15) days
• Documents problems, appropriate goals, interventions, and patient/family response to hospice care

Required Qualifications

• Current unrestricted RN licensure in the state of practice experience
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
• Minimum of two years full-time experience working as an RN. However, two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse

Preferred Qualifications

• Current CPR certification or ability to complete within 90 days of hire
• 1+ years of clinical experience
