Job summary

Amedisys Home Health is seeking a dedicated registered nurse to join our hospice-focused team in Baton Rouge, Louisiana. You will assess patients, coordinate care, and support families using telephonic and in-person visits, with planned weekend shifts and holiday coverage.

Qualifications

• Current, unencumbered RN license in the state you are assigned to work.
• One year of experience as a registered nurse.
• Current CPR certification.
• Comfortable with technology.
• Willing and eligible to obtain additional RN licenses in other states (reimbursed).

Responsibilities

• Assesses physical, environmental, and emotional factors telephonically to determine hospice needs.
• Utilizes EHR to develop recommendations and plan of care.
• Provides patient/caregiver education and hospice support via phone and visits.
• Collaborates with pharmacies, DME vendors and other agencies for patient management.
• Facilitates delivery or maintenance of provided medical equipment.
• Assigns visits, admissions and follow-up calls to on-call staff.
• Documents in real-time to promote continuity of care.
• Participates in performance improvement initiatives.
