Job summary

Elara Caring is seeking a Registered Nurse to join our team and deliver compassionate, high-quality home health care. You will assess patients, coordinate plans of care, educate families, and collaborate with physicians to ensure the Right Care, at the Right Time, in the Right Place.

We value bedside manners, critical thinking, and the ability to manage multiple priorities in a fast-paced home setting. A state RN license, 1+ year of clinical experience, and reliable transportation are required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.

Responsibilities

• Assess patients’ physical, psychological, social, and spiritual needs and incorporate findings into the Plan of Care.
• Complete initial evaluations and reassess nursing needs during follow-up visits.
• Monitor patient condition and communicate changes to the physician and care team.
• Update the Plan of Care as patient needs change and coordinate revisions.
• Educate patients, families, caregivers, and staff on the Plan of Care and pain management.
• Collaborate with the Medical Director and care team to coordinate patient care.
