Job summary

TFI Family Services in Kansas is hiring a full-time Aftercare Case Manager to support children and families who have achieved permanency through reintegration, adoption, or aging out.

You will focus on risk management, crisis intervention, and coordinating community services, meeting families in person to promote reunification and lasting stability. A bachelor's in human services and a valid driver's license are required, with ongoing training provided.

Responsibilities

• Focus on risk management and help families maintain stability during the aftercare period.
• Provide crisis intervention and work with public and private agencies offering supportive services.

• Meet with families in-person to support reunification and resolve any concerns.
