Job summary

Emblem Hospice in Phoenix, AZ is seeking a Hospice Triage RN for after-hours coverage on a 7-on/7-off schedule, including overnights and weekends. You will serve as the clinical anchor, coordinating care with day teams and ensuring timely, compassionate patient visits throughout the service area.

You’ll triage needs, document diagnoses, perform wound care per plan, educate families, obtain physician orders, and coordinate meds and supplies while upholding quality and compliance.

Qualifications

• Arizona RN license required.
• Hospice or home health experience preferred.
• CPR certification required.
• Strong communication and clinical judgment.

Responsibilities

• Coordinate after-hours hospice care and patient intake.
• Conduct patient visits and gather clinical information for RN Case Managers.
• Perform treatments including wound care per plan of care.
• Triage needs, determine nursing diagnoses, support plan-of-care implementation.
• Educate patients and families regarding medications and care needs.
