Job summary

Enhabit Home Health & Hospice in Alabama is seeking an experienced Registered Nurse to coordinate patient care in home settings, assess needs, and implement the interdisciplinary plan of care.

The role requires an active RN license in the state, a minimum of one year of RN experience, and ability to use mobile devices for charting. Medicare/home health or hospice experience is preferred, with CPR certification required and clean driving record.

Qualifications

• Graduate of an approved nursing education program.
• Licensed RN in the practicing state.
• Minimum of 1 year RN experience.
• Basic technology skills including operation of a mobile device.
• Medicare home health or hospice experience preferred; CPR certification may be required.

Responsibilities

• Ensure care delivery complies with federal and state regulations.

Responsibilities

Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan. Perform services in accordance with the physician’s orders and the established plan of care (POC). Ensure delivery of care is compliant with the federal and state regulations.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working conditionMust be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
