Job summary

Enhabit Home Health & Hospice is seeking an experienced RN to coordinate patient and family care under the interdisciplinary plan of care. You will perform services per physician orders and ensure compliance with care plans and regulations.

Requirements include an approved nursing education, an active RN license in the state, and at least one year of RN experience. Strong tech skills, including using a mobile device, are expected.

Qualifications

• Must be a graduate of an approved school of nursing education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed registered nurse (RN).
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Medicare home health or hospice experience is preferred.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
