Job summary

Elara Caring in Springfield, IL is seeking an After-Hours/On Call Registered Nurse to provide compassionate hospice care outside regular hours. You will assess patient needs, monitor conditions, and support families within the interdisciplinary Plan of Care.

The role requires an active RN license, nursing degree, at least one year of clinical experience, and ability to lift up to 50 pounds. Travel to patient homes is required; driving conditions and hospice/home health experience are preferred.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associates or Baccalaureate).
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and staff in following the Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
