Job summary

Gentiva Hospice is seeking an After Hours RN in Columbus, Ohio to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate home-based care. The role reports to the Administrator or Patient Care Manager and focuses on labors after normal hours with coordination across the hospice team.

The ideal candidate holds an active RN license in Ohio, has at least 1 year of nursing experience, and demonstrates a hospice-heart with strong clinical judgment.

Qualifications

• Active RN license in the state of employment (or eligibility)
• Minimum 1 year of nursing experience
• Hospice heart: compassionate, calm, and dependable in urgent care situations
• Excellent clinical judgment and ability to provide support in emotionally intense settings
• Comfortable providing care in patients' homes, facilities, or wherever they reside

Responsibilities

• Provide direct patient care and support after hours
• Serve as liaison between patients, families, physicians, and the hospice team
• Respond promptly to urgent calls, visits, and symptom management needs
• Evaluate patient and family needs to guide clinical decisions
• Document clinical visits and communications in the EMR
• Participate in quality assessment and performance improvement programs
• Work independently while coordinating with the daytime care team
