Puget Sound Home Health & Hospice is seeking a Full-Time After-Hours Hospice Registered Nurse (RN) Case Manager to cover King and Pierce counties, WA. The role involves autonomous nursing visits, triage, and symptom management during evenings and overnights.

You will collaborate with the interdisciplinary team to ensure high‑quality patient care and education for families. Qualified candidates hold WA RN license, BLS, and hospice experience; DOE compensation ranges from $105,000 to $120,000

Qualifications

• Must hold a Washington State RN license.
• Current AHA BLS certification.
• Hospice agency experience preferred or ability to train.
• Ability to work autonomously during evening, night, and early morning hours.

Responsibilities

• Deliver after‑hours hospice nursing visits and urgent care as needed.
• Triage calls, determine interventions, and perform in‑person visits.
• Manage pain and symptom control per hospice standards.
• Educate patients and families on hospice care and medications.
• Document assessments and interventions accurately and promptly.
• Coordinate with the IDT for continuity of care.
• Participate in QAPI and quality improvement initiatives.
