Job summary

Heartland Hospice is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care and peace of mind.

You will work as the primary liaison among patients, families, physicians, and the hospice team, provide direct care after hours, document visits, and support a multidisciplinary team while ensuring continuity of care.

Qualifications

• Current RN license in Missouri or eligibility to obtain.
• 1+ years of nursing experience.
• Hospice heart: compassionate, calm, and dependable in urgent care.
• Excellent clinical judgment and ability to support in emotional settings.
• Comfort providing care in patients' homes or other settings.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.

Responsibilities

• Respond to patient needs during evenings, nights, weekends and holidays.
• Provide direct patient care and support after normal business hours.
• Serve as liaison between patients, families, physicians and hospice team.
• Respond promptly to urgent calls, visits and symptom management needs.
• Evaluate needs and implement the plan of care.
• Collaborate with LPNs/LVNs, CNAs and other staff.
• Document clinical visits and communications.
• Participate in quality assessment and performance improvement.
• Work independently while coordinating with daytime care team.
