Job summary

Anchor Health is seeking an After-Hours Hospice LVN/RN to provide skilled on-call nursing care outside standard hours, primarily evenings, nights and weekends in Vallejo, CA. You will work independently while coordinating with the interdisciplinary team to ensure individualized care plans and symptom management for hospice patients and families.

The role emphasizes compassionate, autonomous clinical judgment, timely documentation, and adherence to hospice standards.

Qualifications

• Current RN or LVN license in good standing.
• Hospice or home health experience preferred.
• Strong clinical judgment and ability to work independently.
• Excellent communication and problem-solving skills.
• Compassionate presence in emotionally sensitive situations.
• Valid driver’s license.
• Reliable, insured vehicle as required by state and organization.

Responsibilities

• Respond to after-hours patient calls and urgent care needs.
• Perform skilled nursing assessments and symptom management.
• Provide emotional support and education to patients and families.
• Implement and update patient plans of care as needed.
• Communicate patient condition changes to the interdisciplinary team.
• Collaborate with physicians and caregivers regarding patient needs.
• Complete accurate and timely clinical documentation.
• Uphold hospice philosophy, policies, and regulatory standards.
