Graham Healthcare Group seeks an After Hours Registered Nurse (RN) for Hospice in West Reading, PA. The role is 40 hours per week (8-hour shifts) with travel to patient homes and a $10,000 first-year bonus, focusing on end-of-life home care with a dedicated team.

Qualifications

• Active Registered Nurse, RN license in the state you work.
• Two years of RN experience and hospice experience preferred.
• Valid driver’s license and ability to travel ~20% of time.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting.
• Respond to after-hours needs with urgent assessments, symptom and pain management.
• Support and guide patients and families during evenings, nights, weekends, and crises.
• Apply clinical judgment to coordinate care and maintain EMR documentation.
• Work independently, but collaborate with teams as needed.
• Travel to patient homes within assigned territory.
