Job summary

Providence at Home with Compassus is seeking an On Call LPN/LVN to provide holistic hospice nursing care under the supervision of a Registered Nurse in the High Desert area. The role supports patients and families, anticipates changes in condition, and ensures continuity of care with a focus on quality of life at the end of life.

Qualifications

• Education and licensure requirements as per State Specific Requirements.
• Experience as LVN in home health preferred.
• Knowledge of hospice and palliative care practices preferred.

Responsibilities

• Provide LPN/LVN hospice nursing services under RN supervision.
• Observe and report patient data to the RN for Plan of Care.
• Implement nursing interventions per Plan of Care and hospice policies.
• Educate patients and families about care needs and changes in condition.
• Document observations, interventions, and communications accurately.
• Collaborate with hospice team to address patient needs and priorities.
