Job summary

A Path of Care Preferred Pathways in Norman, OK is seeking a full‑time ADvantage Home Care RN Case Manager. This in‑home role focuses on coordinating care plans, performing nursing assessments, and guiding families while maintaining professional boundaries and compassionate care.

You will travel to members’ homes, work Monday–Friday, 8:00 AM–5:00 PM, hold an active Oklahoma RN license, CPR within 60 days, and maintain reliable transportation and a valid driver's license.

A Day in the Life of an ADvantage Home Care RN Case Manager

As an ADvantage Home Care RN Case Manager, you play a key role in ensuring members receive the services they need to remain safely in their homes. Your responsibilities include:
