Job summary

Preferred Pathways is hiring a full-time ADvantage Case Manager to provide in-home care coordination in the Lawton, OK area. You will assess needs, develop care plans, and monitor progress to help members live independently at home.

Role requires an Oklahoma RN or LPN license, a Bachelor's degree, and driving ability for long-distance travel. CPR within 60 days for RN/LPN and a valid driver’s license are mandatory.

Qualifications

• Oklahoma RN or LPN license required.
• Bachelor's degree and one year paid professional experience with aging or disabled population.
• CPR certification within 60 days of hire (for RN/LPN only).
• Valid driver's license and a good driving record. Reliable transportation and proof of insurance. Ability to travel long distances.
• Experience with the Advantage Program is preferred.

Responsibilities

• Perform initial assessments of the member's needs and develop plans of care.
• Conduct monthly monitoring and coordinate services to keep members safe at home.
• Initiate preventive interventions and participate in in-service programs and Advantage training.
