Responsibilities

• Develop written plans of care based on nursing diagnoses.
• Assist patients and families in developing health care goals.
• Determine priorities of patient care based on needs and resources.
• Provide nursing interventions safely and competently.
• Offer telephone consultation to patients and families.
• Anticipate changes in patient status and modify care plans accordingly.
• Collaborate with healthcare team in planning and implementing care.
• Assess needs for hospitalization and initiate discharge planning.
• Develop educational components for nursing care plans to encourage compliance.
