Job summary

Sevita in Arizona is seeking an Occupational Therapist (OT) to deliver comprehensive evaluations and develop personalized treatment plans across clinic and community settings, with opportunities for telehealth. The role offers flexible scheduling, a diverse caseload, and collaboration with an interdisciplinary team across PTs, OTs, and SLPs.

Qualifications

• Master's Degree in Occupational Therapy.
• Arizona OT license in good standing.
• At least one year of experience in human services.
• Pediatric experience or assistive technology experience preferred.
• Valid driver's license, vehicle registration, and auto insurance.
• Excellent communication and collaboration skills.
• Passion for helping individuals reach their highest level of independence.

Responsibilities

• Completing comprehensive occupational therapy evaluations.
• Developing individualized treatment plans to promote independence and quality of life.
• Providing engaging therapy services in clinic, community, and telehealth settings.
• Monitoring progress and modifying treatment plans as needed.
• Collaborating closely with physicians, families, caregivers, educators, and interdisciplinary team members.
• Conducting home and community assessments when appropriate.
• Maintaining accurate and timely clinical documentation.
• Supervising Certified Occupational Therapy Assistants (COTAs), when applicable.

What You'll Do

What We're Looking For
