Job summary

UMass Memorial Health is seeking an Advanced Therapeutics clinical pharmacist to provide patient-centered services, including comprehensive medication reviews and adherence monitoring. You will design targeted medication/disease management programs and oversee MTM and transitions of care with pharmacy technicians.

The role requires an ASHP-accredited residency and a PharmD, with board certification preferred.

Qualifications

• Requires Doctor of Pharmacy degree and completion of an ASHP-accredited residency or equivalent experience.
• Nationally recognized certification (e.g., board certification) in the specialty when available.

Responsibilities

• Provide patient-centered clinical pharmacy services, including comprehensive medication reviews and adherence monitoring.
• Design and implement targeted medication and disease management programs.
• Supervise MTM and transitions of care with pharmacy technicians.
• Maintain accurate patient medication profiles and document interventions in the EHR.
