UMass Memorial Health is seeking an Advanced Therapeutics clinical pharmacist to provide patient-centered services, including medication reviews, adherence monitoring, and targeted medication reviews. The role designs and implements disease management programs and collaborates with the care team for optimized clinical and financial outcomes.

The position reports to an immediate supervisor, with responsibilities spanning MTM, documentation in EHR, and supervision of pharmacy technicians.

Competenze

• Doctor of Pharmacy degree and ASHP-accredited residency.
• Nationally recognized certification in area of specialty, when available.
• ASHP PGY2 residency plus 1 year specialty OR PGY1 plus 3 years OR 5 years of practice in specialty.
• Completed a project management course or led a major project in specialty.

Mansioni

• Maintains an active practice in a specialized or focused area.
• Contributes to clinical services, cost containment or process improvement.
• Leads hospital interdisciplinary committees or taskforces in a focused practice area.
• Develops and maintains clinical practice guidelines and SOPs.
• Supports research projects and quality improvement initiatives.
