Job description

STGi is currently seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide services at our Community Based Outpatient Clinic. The general duty of the NP or PA is to provide professional clinical services under the direction of the Community Based Outpatient Clinic (CBOC) Primary Care Provider (PCP). The NP or PA is an Advanced Practice Generalist who offers a wide range of health services, including urgent and emergent evaluation of episodic and chronic problems, and comprehensive, integrated primary health care addressing most of the client's ongoing health needs. They are knowledgeable in assessment, treatment, evaluation of health problems, emergency measures, preventive health, medication prescribing, ordering and interpreting diagnostic tests, and patient education, utilizing community, social, and health resources. The NP or PA maintains current practice knowledge, acts as a role model and mentor, advocates for clients, and participates in professional activities at local and state levels.

ESSENTIAL FUNCTIONS:
  1. Provides primary and preventive medical care within a limited scope of CPT codes, aligned with their skills and training.
  2. Delivers medical care according to standards established by recognized credentialing organizations for Nurse Practitioners and/or Physician Assistants.
  3. Operates within the scope of practice defined by state regulations, the Joint Commission standards, and the Office of the Inspector General.
  4. Works closely with the Patient Aligned Care Team (PACT) and support staff, including daily team huddles.
  5. Focuses on wellness, prevention, and early detection.
  6. Supervises support staff involved in patient care delivery.
  7. Maintains universal precautions and infection control practices.
  8. Ensures smooth and timely patient flow according to VA access standards and triage protocols.
  9. Performs CPR and assists during respiratory and cardiac arrest procedures.
  10. Gathers and assesses health history to investigate health problems or needs.
  11. Performs physical examinations appropriate to patient history.
  12. Diagnoses health issues based on history, exam findings, and diagnostics.
  13. Develops tailored treatment plans, including health education and pharmacologic/non-pharmacologic treatments.
  14. Prescribes medications as appropriate.
  15. Evaluates treatment outcomes and modifies plans as necessary.
  16. Makes referrals for specialty evaluations or treatments.
  17. Coordinates primary care services for walk-in patients.
  18. Models clinical practice within the clinic setting.
  19. Sets up and operates therapeutic and emergency equipment (monitor, EKG, oxygen, AED, suction).
  20. Obtains patient history, records findings per VA standards.
  21. Performs physical exams within privileges and records findings.
  22. Orders diagnostic tests based on presenting complaints and findings.
  23. Demonstrates knowledge of pharmacology.
  24. Prescribes and administers medications within privileges and regulations.
  25. Recommends and follows up on laboratory and diagnostic procedures.
  26. Documents all patient information accurately in medical records and VISTA system.
  27. Keeps VISTA clinic reminder system current.
  28. Closes patient charts within 24-72 hours and places RTC orders promptly.
  29. Assists in scheduling appointments per VA standards and medical necessity.
  30. Informs patients of their care plans, including test results.
  31. Provides educational materials and resources to patients and families.
  32. Ensures electronic documentation is complete and accurate.
  33. Complies with safety and operational regulations (federal, state, Joint Commission, OSHA, VA, subcontractors).
  34. Participates in Performance Improvement programs and peer reviews.
  35. Completes reports and clinical information accurately and timely.
  36. Participates in staff development programs.
  37. Performs other clinical duties within privileges.
  38. Maintains confidentiality and supports patient privacy and safety.
  39. Aids in managing daily clinic activities.
  40. Performs other work-related duties as assigned.
