Job summary

Mercy is seeking an RN II Advanced Primary Care to provide virtual care for chronic disease management. You will build patient relationships, monitor vital trends, and conduct visits between office appointments as part of a multidisciplinary team led by an Advanced Primary Care Lead APP.

Responsibilities include panel management, personalized care, and collaboration with physicians and APPs to improve outcomes.

The RN II Advanced Primary Care role provides virtual care to patients that benefit from ongoing chronic disease management. In partnership with a multidisciplinary team, including physicians and advance practitioners, the RN is responsible for creating patient relationships, monitoring vital sign trends, conducting patient visits in between regular office visits, assisting in panel management of patients and providing individualized care to improve patient outcomes. Reports to respective Advanced Primary Care Lead APP.

Licensure and Certifications

Is personally responsible for obtaining, and maintaining, a current RN license within the hiring state and/or compact licensure in which nursing duties are performed and must meet all state board of nursing requirements.

Must be eligible for licensure in other states.

Education and Experience

Graduation from an accredited registered nursing program

Bachelor’s degree in nursing (BSN) or equivalent work experience

Minimum 5 years of healthcare experience required.

Previous telemedicine, internal/family medicine, or care management experience preferred.
