Job summary

Mercy is seeking an RN II Advanced Primary Care to deliver virtual care for patients with chronic disease management. You will work with a multidisciplinary team including physicians and Advanced Practice Providers to build patient relationships and monitor health trends.

You will conduct visits between office appointments, participate in panel management, and provide individualized care to improve outcomes.

Qualifications

• Graduation from an accredited registered nursing program.
• Bachelor’s degree in nursing (BSN) or equivalent work experience
• Minimum 5 Years Of Healthcare Experience Required
• Previous telemedicine, internal/family medicine, or care management experience preferred.

Responsibilities

• Provide virtual care for chronic disease management.
• Create patient relationships and monitor vital sign trends.
• Conduct patient visits in between regular office visits.
• Assist in panel management to improve patient outcomes.
