Job summary

Mercy is seeking an RN II for Advanced Primary Care to provide virtual care and ongoing support for patients with chronic conditions. You will work with a multidisciplinary team to monitor vitals, conduct virtual visits, and coordinate care across ambulatory settings.

The role involves advancing GDMT, discharge follow-up, and collaboration with physicians and advanced practitioners to improve patient outcomes, with hybrid work after training.

Qualifications

• Active RN license in applicable state(s).
• Experience in chronic disease management and virtual care preferred.
• Strong communication and collaboration skills.

Responsibilities

• Build strong patient relationships and monitor vitals.
• Conduct virtual visits between office appointments.
• Support chronic disease management, discharge follow-up, and acute care needs.
• Advance GDMT and facilitate advance care planning for Medicare/MA patients.
• Coordinate care with primary care clinicians in ambulatory settings.
• Perform virtual assessments, patient education, triage, and escalation of care.
