Palmetto Primary Care Physicians (PPCP) is seeking a dedicated and compassionate Advanced Practitioner (Nurse Practitioner or Physician Assistant) to join our healthcare team. Essential Duties and Responsibilities
  • Conduct comprehensive and focused physical examinations, health histories, and risk-factor assessments for patients of all ages.
  • Diagnose and manage acute illnesses, injuries, and chronic conditions (e.g., diabetes, hypertension, asthma, obesity, depression/anxiety) within scope of practice.
  • Order, perform, and interpret diagnostic tests and imaging studies (labs, EKGs, point-of-care testing, etc.).
  • Prescribe medications and therapeutic interventions in accordance with state regulations and collaborative practice agreements.
  • Develop, implement, and monitor individualized treatment and care plans, adjusting as needed based on patient response.
  • Provide preventive care services including immunizations, screenings, well visits, and health risk assessments.
  • Educate patients and families on disease prevention, treatment plans, medication management, and lifestyle modifications.
  • Refer patients to specialists or ancillary services as clinically indicated and coordinate follow-up care.
  • Document patient encounters accurately and timely in the electronic health record (EHR), maintaining compliance with legal and regulatory documentation standards.
  • Collaborate with supervising/collaborating physicians on complex cases requiring additional expertise.
  • Respond to patient messages, prescription refill requests, and lab/test result follow-up in a timely manner.
  • Participate in care coordination for chronic disease management, transitions of care, and population health initiatives.
  • Maintain up-to-date knowledge of clinical guidelines, best practices, and evidence-based protocols relevant to family medicine.
  • Adhere to all clinic policies, procedures, and regulatory/compliance requirements (HIPAA, OSHA, etc.).
  • Participate in quality improvement initiatives and performance metrics tracking.
Non-Essential Duties and Responsibilities
  • Participate in community health outreach events, health fairs, or educational seminars.
  • Precept or mentor students, residents, or new APs as needed.
  • Serve on clinic or organizational committees.
Requirements
Required Knowledge, Skills, and Abilities (KSAs)
Knowledge:
  • Comprehensive knowledge of family medicine practices, including pediatric, adult, and geriatric primary care.
  • Strong understanding of pharmacology, disease processes, and evidence-based clinical guidelines.
  • Familiarity with preventive care standards, chronic disease management protocols, and screening guidelines.
  • Knowledge of applicable state and federal regulations governing AP practice, prescribing authority, and scope of practice.
  • Working knowledge of electronic health record (EHR) systems and clinical documentation standards.
Skills:
  • Strong clinical assessment, diagnostic reasoning, and problem-solving skills.
  • Excellent verbal and written communication skills for patient education and interdisciplinary collaboration.
  • Proficient in performing common outpatient procedures (e.g., wound care, injections, biopsies, as applicable).
  • Time-management and organizational skills to manage a full patient panel efficiently.
  • Proficiency with EHR and clinic scheduling/documentation software.

  • Ability to work both independently and collaboratively within a care team.

Required Qualifications:
  • Physician Assistant: Bachelor’s degree and successful completion of accredited physician assistant program.
  • Nurse Practitioner: Master’s degree and certification as a Certified Registered Nurse Practitioner (CRNP) based on RN licensure and state authority for advanced practice.
  • Current, unrestricted state licensure as a Nurse Practitioner or Physician Assistant.
  • National certification (e.g., ANCC/AANP for NPs; NCCPA for PAs).
  • Current DEA registration or eligibility to obtain one.
  • Current BLS certification.
  • Minimum of 1–2 years of family medicine or primary care experience preferred (new graduates may be considered).

  • Ability to exercise independent clinical judgment and critical thinking, often under time constraints.
  • Ability to manage stress associated with high patient volumes, urgent situations, and emotionally sensitive conversations.

  • Ability to prioritize competing tasks and adapt quickly to changing clinical priorities.
  • Emotional resilience when managing patients with complex psychosocial needs, chronic illness, or end-of-life concerns.
Physical Requirements
  • Ability to stand, walk, and move throughout the clinic for extended periods during a shift.
  • Ability to sit for periods of time while documenting in the EHR.
  • Ability to lift and/or assist patients (up to 25–50 lbs) as needed for examinations or procedures.
  • Manual dexterity and visual acuity sufficient to perform physical exams, procedures, and use of clinical instruments.
  • Ability to hear and communicate clearly, in person and by phone, for patient care and team collaboration.
  • Occasional bending, reaching, kneeling, or twisting to perform examinations or procedures.

This position is based in an outpatient family medicine clinic. The role involves:

  • A fast-paced clinical environment with direct patient contact throughout the workday.
  • Regular exposure to communicable diseases, bodily fluids, and other conditions common to a clinical/patient care setting, requiring adherence to standard precautions and infection control protocols.
  • Use of standard clinical and office equipment (EHR systems, diagnostic tools, phones, computers).
  • Frequent interruptions and the need to manage multiple patients and tasks simultaneously.
  • Collaborative interaction with physicians, nursing staff, medical assistants, and administrative personnel.
  • Compliance with clinic safety, infection control, and OSHA/CLIA regulatory standards.
  • Noise levels typical of a busy outpatient clinic (moderate).
Summary

An exempt position responsible for delivering comprehensive, patient-centered primary care to individuals and families across the lifespan in an outpatient clinic setting. Working collaboratively with physicians, nursing staff, and clinic support personnel, the AP independently or collaboratively evaluates, diagnoses, and manages acute and chronic health conditions; provides preventive care and health education; and coordinates care with specialists and community resources. The role requires sound clinical judgment, strong communication skills, and a commitment to delivering high-quality, evidence-based care within a fast-paced environment.
