We are seeking an experienced Advanced Practice Registered Nurse (APRN) to join our nephrology team and provide comprehensive care to patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD).
This is a dynamic clinical role with a strong outpatient nephrology focus, while also supporting patients across dialysis centers and hospital settings. Working under the direction of our Medical Director and in close collaboration with our nephrologists, the APRN will help ensure continuity of care across the patient's full kidney-care journey.
The ideal candidate combines strong clinical judgment, nephrology knowledge, independence, and excellent collaboration skills with a genuine commitment to improving patient outcomes. Given the role's involvement in hospital-based nephrology care, candidates with acute-care experience, including experience managing medically complex patients in ICU or telemetry settings, are especially encouraged to apply.
This position provides direct patient care across outpatient nephrology practices, dialysis centers, and hospital environments. The role requires regular travel between assigned local locations and may involve prolonged periods of standing, walking, bending, and computer use, as well as occasional assistance with patient mobility and clinical equipment.
Exposure to infectious materials may occur in the course of patient care. Appropriate PPE is provided and required.
