Job summary

Metro Healthy Communities in Florida seeks an experienced APRN to screen, diagnose, and treat primary care patients and HIV care for adults. Under medical director supervision, you will collaborate with staff to deliver respectful, high‑quality care in a community health setting.

Requirements include active Florida APRN license and at least five years of outpatient experience; HIV care experience and AA HIV M certification preferred; bilingual English/Spanish strongly preferred.

Qualifications

  • Active Florida APRN license with no encumbrances.
  • Minimum 5 years of outpatient ambulatory primary care experience.
  • Experience providing HIV care is preferred; AA HIVM certification a plus.
  • Bilingual English/Spanish strongly preferred.
  • Excellent communication and teamwork in a community health setting.

Responsibilities

  • Provide high quality comprehensive care to new and established patients under Medical Director supervision.
  • Educate patients on wellness, prevention, and early detection with materials for families.
  • Determine required referrals and coordinate with consultants and subspecialists.
  • Assess urgency of follow-up and arrange follow-up appointments as needed.
  • Formulate care plans, order tests and services, and prescribe appropriate treatment.
  • Document all care in the EMR (eClinicalWorks) in a timely manner.
  • Consult with Medical Director and staff on patient care and education issues.
  • Participate in quality improvement, continuing education, and related programs.
  • Maintain patient confidentiality per standards and METRO policies.
  • Provide after-hours call coverage for urgent patient needs.

Statement of Purpose

This Provider, under the supervision of the Medical Director, is responsible for screening, diagnosing and treating primary care patients and providing HIV care to adults in a sensitive and non-judgmental manner. The Provider will support the organization’s goal in providing primary care and specialty care to both HIV positive and HIV negative individuals. Demonstrated ability to collaborate effectively with colleagues and deliver respectful, high-quality care to all patients in a community health setting across wide range of backgrounds and perspectives. Bilingual (English/Spanish) strongly preferred.


Primary Tasks/Responsibilities


  • Provide high quality comprehensive care and treatment to new and established patients under the supervision of the Medical Director

  • Educates patients on wellness, prevention, and early detection by providing materials and resources to the patients and families.

  • Determines which referrals are required based on examination and patient needs, initiates those linkages appropriately and communicate effectively with consultants and subspecialists.

  • Determines level of urgency of follow-up, referral/consultation appointments.

  • Formulates plan of care, orders studies, tests and ancillary services and prescribes medical treatment for patients, as clinically appropriate.

  • Documents all care in the patient’s medical record, eClinicalWorks, in a timely manner.

  • Consults with Medical Director and other professionals on staff, as needed, regarding patient care, assessment, and education issues.

  • Participates in quality improvement, continuing education, and other patient care programs established by the organization.

  • Maintains confidentiality regarding patient and/or family in accordance with professional and departmental standards.

  • Complies with all established METRO policies, procedures and protocols as well as any department-specific policies and procedures as they relate to provider functions.

  • Maintains current professional license

  • Promotes and practices the mission, vision and values of Metro Healthy Communities.

  • Provides after hours call coverage for patient needs that are of an urgent nature.


Education/Professional


  • Current, Active, and unencumbered Florida State Licensure, APRN

  • Minimum of 5 years of experience providing outpatient ambulatory primary medical care

  • Experience providing HIV care and AAHIVM certification preferred, but not required. If not AAHIVM certified, willingness and ability to obtain this certification within 12 months of starting the position, with training provided on site.


Knowledge, Skills And Competencies Required


  • Knowledge of best practices for working with uninsured and under-served communities

  • Experience in Sexual Health and in screening, treating & patient education in sexually transmitted infection within a harm reduction model of care, preferred.

  • Ability to provide care and treatment to patients diagnosed with co-morbid conditions.

  • Knowledge of and ability to apply professional medical principles, procedures, and techniques.

  • Ability to document in EMR systems.

  • Excellent communication and interpersonal skills.

  • Ability to work as part of a team, maintaining productivity and delivery of high-quality care.

  • Must possess and maintain valid Florida driver’s license and proof of insurance

  • Must have reliable transportation.
