Job summary

Community Health Network seeks an Advanced Practice Provider for its vascular team at Community Heart and Vascular Hospital in Indianapolis, IN. The role requires travel to outside locations to assist with surgeries.

We look for a board-certified APN/PA with 3–5 years in vascular-related roles and eligibility for prescriptive authority. Monday–Friday schedule with on-call rotation and a strong benefits package will be provided.

Qualifications

• 3 to 5 years' experience in vascular-related roles.
• Board certification as an Advanced Practice Provider (APN/PA) with ongoing certification.
• Eligible for prescriptive authority per Indiana State Board of Nursing.

Responsibilities

• Vascular surgery rounding and potential clinic responsibilities at Community Heart and Vascular Hospital.
• Work Monday–Friday, 8:00 a.m. to 5:00 p.m., with surgical on-call rotation (1 in 3).
• No vascular weekend call.
• Utilize Epic EMR for documentation and orders.

This opportunity is located at our Community Heart and Vascular Hospital, at 8075 North Shadeland Avenue, Indianapolis, IN 46256 and will require travel to outside locations to assist with surgeries.

Highlights

Community Physician Network is seeking a board-certified advanced practice provider to join a well‑established vascular team at our Community Heart and Vascular Hospital. We are looking for a highly motivated, experienced Physician Assistant or Nurse Practitioner with required experience in vascular surgery, cardiology, surgical first assist, ER, PCU, or ICU.

• Vascular surgery, rounding, and potential clinic responsibilities at Community Heart and Vascular Hospital.


• Monday through Friday, 8:00 a.m. to 5:00 p.m. schedule. Surgical on‑call rotation (1 in 3). No vascular weekend call.

Experience

• 3 to 5 years' experience required.


• Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification; must maintain certification on an ongoing basis.


• Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; must maintain eligibility for prescriptive authority on an ongoing basis.
