Job summary

One to One Health is seeking TextCare Providers to deliver virtual acute care and ongoing patient support via text, phone, and video from a remote location. You will build trusted patient relationships, coordinate follow-up care, and help improve access and outcomes in a text-centered care model.

Required qualifications include completion of an accredited NP or PA program, active Tennessee NP or PA license (or willingness to apply), prescriptive authority, and BLS certification; at least 1 year

Responsibilities

• Deliver virtual care through text, phone, and video.

• Coordinate referrals, follow-up care, labs, and imaging.
• Maintain continuity of care through proactive communication and follow-up.
• Collaborate with clinical teams to ensure seamless patient experience.
