Endeavor Health in Chicago, IL is hiring an Advanced Practice Provider (APN/PA) for Family Medicine in a full-time outpatient setting. You will work under collaborating physicians, performing exams, ordering imaging and labs, and interpreting results to guide patient care.

The role requires IL APN/PA licensure, DEA license, and BLS/CPR certification, with prior APN/PA experience preferred. Rotating shifts and weekend call are part of the schedule.

Qualifications

• Master's level program in family medicine or primary care.
• APN/PA licensure in Illinois is required; DEA license needed.
• BLS/CPR certification is required.

Responsibilities

• Provide care to patients in outpatient primary care settings.
• Perform history & physicals, order tests, interpret results.
• Prescribe therapies and educate patients on regimens.
• Collaborate with physicians and care teams to improve outcomes.
• Maintain records and participate in quality and safety initiatives.

$57.31 - $72.36 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Job Description

Exciting opportunity to join Endeavor Health in the Primary Care - Family Medicine Department. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
• Education: Completion of master's level program in specialty area
• Certification: BLS/CPR certification required
• Experience : Prior APN/PA experience preferred
