Job summary

Crossroads Health in Northeast Ohio seeks an Advanced Practice Provider (PA/NP/APRN) to deliver primary care in an integrated setting. You will diagnose, treat, and manage patients across the lifespan as part of a multidisciplinary team.

The position requires an Ohio-licensed APP with a master's degree, plus hospital or family practice experience. Employer-provided liability insurance, supervision, and BLS/First Aid are included.

Come and be a part of our Primary Care Health Center with locations in Mentor, Willoughby and Euclid. The Advanced Practice Provider (APP) is a Physician Assistant (PA), Nurse Practitioner (NP), or Advance Practice Registered Nurse (APRN). The APP works in accordance with a standard collaborative care arrangement with a physician or to provide primary care services collaboratively within a multidisciplinary health team. The APP is responsible for providing comprehensive patient-centered care in an integrated healthcare setting. This position is responsible for individual services to patients assigned by Crossroads Health in the Integrated Healthcare Center

Additionally, the Prescriber will:

• Provide general comprehensive primary medical care services to individual patients across the life span in the office setting including behavioral health assessment, medication evaluation and medication management, and prescription of medication in compliance with Ohio state regulations using a trauma informed approach.
• Provide diagnosis, screening, education, treatment, referrals, general medical procedures, and follow-up of services.
• Provide medical coordination of ongoing treatment involved with the transfer or discharge planning of health center patients in various settings.
