Black Women's Mental Health Institute seeks an Advanced Practice Provider (APP) to deliver compassionate care across diverse patient populations in a community hospital setting. The APP will assess, diagnose, and plan treatment while coordinating ongoing care and transitions.

The role emphasizes strong clinical judgment, effective communication, and collaboration with multidisciplinary teams to ensure safe, evidence-based care and patient education for families and patients.

Formación

• Current licensure and certification as an APP (scope of practice consistent with role).
• Strong clinical judgment, communication, and interpersonal skills.

Responsabilidades

• Provide patient assessment, diagnosis, and treatment planning across clinical settings.
• Manage and coordinate ongoing patient care, including follow-up and care transitions.
• Order and interpret diagnostic tests and adjust care based on patient response.
• Collaborate with physicians and multidisciplinary teams to ensure safe, effective care.
• Deliver patient and family education to support informed decision-making.
