Commonwealth Pain and Spine is seeking an Advanced Practice Provider (APP, APRN, PA) in Indiana to join a multidisciplinary pain management team. You will assess patients, develop care plans with physicians, and perform injections and orders for diagnostics and therapies as needed.

Qualifications

• APRN license in Indiana is required to practice.
• DEA registration preferred for prescribing authority.
• Must be able to obtain clinical privileges in the network.

Responsibilities

• Identify and assess patient health care needs and plan care.
• Record findings and formulate prognosis with physician plan.
• Perform evaluations and re-evaluations of patients.
• Refer patients to physicians or specialists as needed.
• Discuss cases with physicians to prepare care plans.
• Perform pain management injections per protocols.
• Prescribe/Order diagnostics, therapeutics, medications, and procedures.
• Work independently and as part of a flexible team.
