Nurse Practitioner (NP) / Physician Assistant (PA)– ALF, ILF & Home Visits
Join our clinical team as a Nurse Practitioner (NP) or Physician Assistant (PA) providing patient-centered care across assisted living facilities (ALFs), independent living facilities (ILFs), and private residences.
This role is ideal for a provider who enjoys building relationships with patients, caregivers, and facility teams while managing both acute and chronic conditions in community-based settings.
What You’ll Do
- Provide in-person medical care to patients in ALFs, ILFs, and private residences.
- Evaluate and manage acute and chronic health conditions and assess changes in patient needs.
- Review patient schedules and prioritize visits based on clinical needs, urgency, patient availability, and travel considerations.
- Prescribe and manage medications, order or approve laboratory testing and diagnostic studies, and coordinate treatments, therapies, equipment, and referrals as clinically appropriate and permitted by law.
- Assess functional status, medication use, care needs, and relevant environmental or social factors when developing and managing the plan of care.
- Communicate with patients, caregivers, facility staff, and the care team regarding findings, recommendations, follow-up needs, and changes in condition.
- Complete timely electronic documentation, orders, visit logs, and charge capture following each visit.
- Participate in provider meetings and assigned after-hours or on-call coverage.
- Maintain patient confidentiality and follow all applicable healthcare, safety, infection-control, and compliance requirements.
What We’re Looking For
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Current, unrestricted NP or PA license in each state of practice.
- Current national board certification applicable to your provider discipline.
- Current DEA registration and controlled-substance authority, when required.
- Current BLS certification.
- Adult or geriatric clinical experience required; community-based or primary care experience preferred.
- Experience with electronic medical records, clinical documentation, coding, and charge capture.
- Reliable transportation and ability to travel between assigned facilities and patient residences.
- Strong communication, organization, clinical judgment, and ability to work independently while knowing when to consult or elevate concerns.
Why This Role?
This is an opportunity to provide meaningful, relationship-based care where patients live while working closely with facility staff, caregivers, and the broader clinical team. If you enjoy a community-based setting and value continuity of care, we’d love to hear from you.
