Job summary

Qualifications

• Master’s degree in a Family Nurse Practitioner program or PA program and active national certification.
• Active, unrestricted state license and multi-state licensure.
• DOT certification and BLS certification required.
• Ability to order and administer vaccines; comfortable with vaccines.
• Ability to work both in-person and virtual care; clinic presence required.

Responsibilities

• Deliver evidence-based primary and family care, including assessment, diagnosis, treatment, and management of acute and chronic conditions.
• Provide both in-person and virtual visits to support positive patient health outcomes.
• Coordinate care with other healthcare professionals and perform preventive counseling.
• Engage patients in wellness services and chronic disease management.
• Manage clinic operations including patient follow‑up and scheduling.
