Job summary

Post Acute Medical Services is seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to care for patients in nursing homes in the Wilbraham, Massachusetts area. Ideal candidates are passionate about delivering high-quality, patient-centered care and possess at least 1 year of professional experience.

The position primarily takes place Monday through Friday and offers a fee-for-service compensation, 25 days of paid time off, and a 401K plan to enhance work-life balance.

About the role:

To take care of patients in the nursing home setting. Long-term care and skilled patients. Working with an MD, diagnosing, treating, and coordinating the care of these patients. We are looking for a full-time, highly motivated and caring provider who's passionate about delivering the highest level of care possible for their patients. The coverage is mainly Monday-Friday position.

Required Qualifications:

• 1+ years of professional experience
• Experience using electronic health record (EHR) software
• Ability to learn/stay up to date on relevant practices
• Critical thinking is needed when issues or concerns arise with patients and families of patients
• Detail-oriented, organized, and with a high degree of accuracy

Preferred qualifications:

• Post acute care/ skilled nursing facility experience
