Job summary

LanceSoft is seeking an experienced Nephrology Nurse Practitioner to join our outpatient nephrology team in Goshen and New Windsor, NY. This clinic-based role focuses on CKD management with physician support and clinical autonomy.

You will manage CKD patients, order and review labs, develop treatment plans with nephrologists, educate patients, and participate in quality improvement. Requires NY NP license, national certification, master's/doctorate, DEA, and 2+ years nephrology experience.

Qualifications

• Active New York State NP license.
• National certification (AANP or ANCC).
• Master’s or Doctorate in Nursing.
• DEA license.
• Minimum 2 years of nephrology experience.
• Preferred: BLS/ACLS certification.

Responsibilities

• Manage and follow patients with CKD, hypertension, electrolyte disorders, and other renal conditions.
• Provide outpatient nephrology care and long-term disease management.
• Order, interpret, and follow up on laboratory studies, imaging, and diagnostic testing.
• Develop and adjust individualized treatment plans with nephrologists.
• Educate patients and families on kidney disease progression and lifestyle changes.
• Coordinate referrals and care with other specialties as needed.
• Maintain accurate EMR documentation.
• Participate in outpatient quality improvement initiatives.

Role Overview:
We are seeking an experienced Nephrology Nurse Practitioner (NP) to join our outpatient nephrology team. This role focuses exclusively on clinic-based kidney care, providing comprehensive management of chronic kidney disease and related conditions in a collaborative, physician-supported environment with strong clinical autonomy.

Key Responsibilities:

• Manage and follow patients with chronic kidney disease (CKD), hypertension, electrolyte disorders, and other renal conditions
• Provide ongoing outpatient nephrology care and long-term disease management
• Order, interpret, and follow up on laboratory studies, imaging, and diagnostic testing
• Develop and adjust individualized treatment plans in collaboration with nephrologists
• Provide patient and family education on kidney disease progression, lifestyle modifications, and treatment options
• Coordinate referrals and care with other specialties as needed
• Maintain accurate, timely, and compliant EMR documentation
• Participate in outpatient quality improvement initiatives
