This care is provided to support optimal health of children within the context of their family, community, and environmental setting. **Essential Functions*** Perform comprehensive histories and physical exams for well and acute visits, identifies normal and abnormal findings, including physical, cognitive and psychosocial status.* Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information.* Develop pharmaceutical and non-pharmaceutical treatment plans.* Monitor effectiveness of plan of care.* Educate patients on disease and treatment plans. Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.* Perform procedures as trained and appropriately credentialed.* Document within the Electronic Medical Record (EMR) and completes other necessary documents within a timely manner.* Collaborate and/or consults with physicians and other health care providers as appropriate as part of the care team.* Participates in organizational initiatives including the development, maintenance and support of the strategic direction of Children’s Wisconsin.
