Job summary

University Hospitals in Ohio seeks an Advanced Practice Provider to deliver on-site care for patients in skilled and long-term care settings, with duties across care planning, assessments, and interdisciplinary collaboration. Job description

Seeking and Advanced Practice Provider for skilled and long term care located at Avon and North Ridgeville. The APP will follow UH patients when they are admitted to a nursing home, providing onsite care and call in the evenings.

A Brief Overview

The Advanced Practice Provider is responsible for providing quality care for patients and increasing patient access and satisfaction. Standard tasks to be performed by a NP/PA as specified by the Ohio State Board of Medicine and UH entity specific Medical Bylaws:

Education Qualifications

(MSN) Master's Degree in Nursing, or
Master's Degree from an accredited PA educational program
