Job description

Riverside Medical Group is currently seeking an Advanced Practice Provider for the night shift labor pool position. This role involves 3 12‑hour shifts per week under the direction of a Cardiothoracic Surgeon within established departmental practice guidelines. The position is located on the campus of Riverside Regional Medical Center in NewportNews.

Responsibilities

• Provide comprehensive management of critically ill cardiothoracic surgery patients in an 18‑bed SICU, functioning autonomously in collaboration with the attending surgeon.
• Conduct complex, standardized patient care procedures requiring experience and specialized training.
• Perform patient evaluations, assessments, and appropriate treatments efficiently.
• Maintain communication with the attending surgeon regarding updates and escalation of care.
• Order and interpret laboratory, diagnostic, and radiographic tests.
• Respond to, coordinate, and manage emergent life‑threatening situations.
• Perform emergent re‑sternotomy, cardiac massage, hematoma evacuation, and temporary hemostasis as needed.
• Demonstrate proficiency in invasive procedures such as chest tube insertion, central venous access, dialysis access, Swan‑Ganz catheter placement, arterial line placement, and DC cardioversion.
• Exhibit extensive knowledge of vasoactive/inotropic drugs, ventilator management, and pacemaker management.
• Experience with ECMO and Impella devices is desirable.
• Expedite and execute transfer of cardiothoracic ICU patients to step‑down units.
• Perform routine rounds on step‑down patients and respond to escalation of care as required.
• Carry the CTS service phone/pager and provide consults in a timely manner.
• Serve as a mentor for nursing staff across the cardiac surgery service line, providing education to newer and novice staff nurses.
• Perform first and second assist in the operating room, including endoscopic vein and radial artery harvest.
• Perform necessary bedside procedures directed by the attending surgeon, such as removal of pacing wires, intra‑aortic balloon pumps, and point‑of‑care ultrasound/echocardiogram.

Qualifications

• Board‑certified Physician Assistant or Nurse Practitioner.
• Current ACLS/BLS certification.
• Current DEA license.
• Strong oral and written communication skills.
• Collaborative attitude and teamwork skills.
• Minimum 4 years of experience with first and second assist for all types of cardiothoracic surgical procedures.
• Proficiency in endoscopic vein harvest (Maquet system) and preferred experience with endoscopic radial artery harvest.
• Inpatient management of intensive care and step‑down patients shared with hospital‑based APPs.
• Documentation in Epic electronic health record.
• Strong theoretical background in anatomy, physiology, general medicine, pulmonary, emergency, and critical care medicine, as well as therapeutics, pharmacology, and interpersonal dynamics.
