Job summary

University of Florida Jacksonville Physicians, Inc. is seeking an Advanced Practice Provider I (APP I) to deliver direct patient care in Primary Outpatient Care within Community Health & Family Medicine and General Pediatrics, under physician supervision.

May perform minor primary care procedures. The APP I obtains histories, performs exams, orders and interprets tests, develops health plans, counsels patients, and documents encounters in MyChart and EMR according to compliance guidelines;

Qualifications

• Masters Nurse Practitioner required or Masters Physician Assistant program required.
• BLS required at time of hire.
• Experience: 2 years in outpatient setting.

Responsibilities

• Obtains patient histories and performs physical examinations.
• Orders, interprets, and evaluates diagnostic tests.
• Develops comprehensive health management plans with physicians, pharmacists, and other medical professionals; includes progress notes, discharge summaries, consults, etc.
• Educates and counsels patients/families regarding health care and routine health maintenance.
• Documents encounters and closes open encounters in the EMR (MyChart) per institutional requirements.
• May provide education and instruction.
• Call coverage may be required depending on departmental needs.
• All other duties as assigned.
